The Surgical Critical Care Fellowship is an ACGME/RRC accredited training program, offered through the University of San Francisco- Fresno, General Surgery Program. This one year fellowship is based out of Community Regional Medical Center (CRMC) in Fresno, California. CRMC is the only Level 1 trauma center between Los Angeles and Sacramento, with 650 beds (68 ICU beds). This catchment area within the San Joaquin Valley includes an approximate area of 15,000 square miles and a population of approximately 4 million, resulting in more than 2,800 annual trauma-related visits to the Emergency Department. Approximately 10-20% of patients are severely injured (Injury Severity Score > 16) and 35% go directly from the ED to the OR or the ICU.

Although the clinical care and didactic instruction are the backbone of this experience, the Fellow concurrently participates in research and administrative activities. The Fellow is expected to take advantage of the wealth of clinical and trauma registry data available for clinical research. Facilities and opportunities for laboratory investigations are also available. It is anticipated that this fellowship experience will develop a well-rounded surgeon who has the tools for leadership positions in Surgical Critical Care/Trauma Surgery. Upon successful completion the fellow is eligible for the ABS Surgical Critical Care Examination. There are ten core faculty who are critical care board certified and active in all aspects of trauma, surgical care, and emergency general surgery. We encourage qualified surgical residents, with an interest in the areas of surgical critical care, education, and research, to consider this exciting opportunity.

Research opportunities

The Surgical Critical Care Fellow is encouraged to take advantage of the wealth of clinical and trauma registry data available for clinical research. Collaboration with critical care core faculty, as well as other UCSF departments (including Emergency Medicine and Neurosurgery), CRMC departments (Pharmacy, Respiratory Therapy) allow for extensive research opportunities. Fellows are required to undertake research projects to be presented at national conferences and published in reputable medical journals. Our surgical research has been presented at the American Association for Surgery of Trauma, Southwestern Surgical Congress, American Burn Association, Western Trauma Association, The Society for Critical Care Medicine and the American College of Surgeons. The UCSF Fresno Department of Surgery has a dedicated research assistant who assists with the preparation of IRB applications, data collection and statistical analysis.
