Statement:
Mesenteric ischemia is a clinical entity that is commonly encountered by the acute care surgeon, but it presents numerous challenges. The diagnosis of mesenteric ischemia is often not straightforward and can cause a diagnostic dilemma in an often critically ill patient. Furthermore, as technology has advanced, operative management of the underlying flow lesion has evolved and new methods of determining what bowel may be threatened have been developed. While there are several cohort studies and some systematic reviews addressing these issues in the literature, there is no rigorous practice management guideline using GRADE methodology to help the acute care surgeon diagnose and appropriately manage mesenteric ischemia. For this reason, we propose the development of a practice management guideline to answer questions on diagnosis of mesenteric ischemia, operative and nonoperative management strategies for mesenteric ischemia, and nuances of management of nonocclusive mesenteric ischemia.
